You can keep the office information for future reference or return to the website at any time to review it.

Payment is expected at the time of service. We are happy to generate and submit a form to your insurance company so you can be reimbursed directly. Please note that Dr. Hoch is not directly on HMO panels and is not a Medicare provider. He is, however, an out of network provider for most insurance plans. Be sure to check with your health plan to see what reimbursement you are entitled to under your specific policy.

Typically reimbursement for out of network care is 80% but it may be different for your plan, and deductible amounts may also be required by your plans before payments are made back to you. Unfortuneately, Medicare may not be billed for our services.

Working under the old "fee for service model" enables Dr. Hoch to provide exceptionally high quality care for your unique health needs. You receive the time you need during office visits (typically 90 minutes of face to face time on your first visit and 30 to 60 minutes at other visits). You get to work with a doctor who knows you and cares about you without interference from a nameless and faceless third parties.
